Initial assessment form
This is a Word document you can download and print out, to use in your initial interviews with Deaf and hearing impaired students. Students may fill this in on their own, or together with the co-ordinator. There is space for co-ordinators to write their own notes based on discussion of the questions with students.

This is a document you can download and print. When you meet with a student and discuss either referring them to the Advance Centre or liaising with the Advance Centre this form needs to be used. It is a general release form which allows us to share relevant information regarding the student.

Research supports the idea that certain skills are helpful for this position: above average typing skills, the ability to “hear” words phonetically, and suitable English skills.
